Jurisdiction
Drives the statutory cite block and approximate response window. For any state not listed, the letter falls back to a generic FLSA recordkeeping reference (29 U.S.C. § 211(c); 29 C.F.R. Part 516).
Records Requested
Used for W-2/1099/payroll scope. Be specific to keep the response manageable.
Format & Purpose
Optional, but a brief purpose statement helps HR scope the response. You are not legally required to disclose your reason in most jurisdictions.
