Letter Inputs

Employee (You)

Employer

If you know the HR director's name, include it on the first line. Otherwise "Attn: Human Resources" is fine.

Employment Period

Leave blank if currently employed.

Jurisdiction

Drives the statutory cite block and approximate response window. For any state not listed, the letter falls back to a generic FLSA recordkeeping reference (29 U.S.C. § 211(c); 29 C.F.R. Part 516).

Records Requested

Used for W-2/1099/payroll scope. Be specific to keep the response manageable.

Format & Purpose

Optional, but a brief purpose statement helps HR scope the response. You are not legally required to disclose your reason in most jurisdictions.
